Resumen

RESUMEN. Antecedentes: Porphyromonas gingivalis  es un patógeno cuyo principal factor de virulencia son las fimbrias, de las cuales existen 6 genotipos (fimA I-V y IB). Conocer su frecuencia y la de otros microorganismos del complejo rojo y Aggregatibacter actinomycetemcomitans en pacientes diabéticos y no diabéticos permite determinar el papel de la microbiota en la fisiopatología de la periodontitis en el diabético. Objetivo: Determinar la frecuencia de 6 genotipos de fimA de Porphyromonas gingivalis y de Tanerella forsythia, Treponema denticola  y Aggregatibacter actinomycetemcomitans en pacientes diabéticos y no diabéticos con periodontitis.  Métodos: Se seleccionaron 99 pacientes con periodontitis, sanos sistémicamente, y 105 pacientes diabéticos con periodontitis, a quienes se le tomaron muestras de Fluido Gingival Crevicular y se utilizó la técnica de PCR para la detección de los genotipos de fimA y otros periodontopatógenos. Los datos se analizaron con la prueba de Chi cuadrado y Kruskal Wallis. Resultados: Porphyromonas gingivalis resultó positiva en 72,7% de pacientes no diabéticos y 28,6% de pacientes diabéticos. El genotipo más frecuente en ambos grupos es fimA II, encontrándose en menor porcentaje en los pacientes diabéticos (P= 0,000). Hubo una alta correlación entre las muestras positivas para Porphyromonas gingivalis y Aggregatibacter actinomycetemcomitans, Treponema denticola y Tanerella forsythiaConclusión: La frecuencia de Porphyromonas gingivalis, Treponema denticola, Tanerella forsythia y Aggregatibacter actinomycetemcomitans es menor en los pacientes diabéticos con periodontitis. El genotipo más frecuente en ambos grupos es fimA II. En general, la distribución de los genotipos de fimA, fue heterogénea, y no hubo diferencias estadísticamente significativas.   

ABSTRACT. Background: Porphyromonas gingivalis is a pathogen whose main virulence factor are the fimbriae, of which there are 6 genotypes (fimA I-V and IB). Know its frequency and other microorganisms Red complex and Aggregatibacter actinomycetemcomitans in diabetic and non-diabetic patients to determine the role of microbiota in the pathophysiology of periodontitis in diabetics. Objective: To determine the frequency of 6 genotypes of Porphyromonas gingivalis FimA and Tanerella forsythia, Treponema denticola and Aggregatibacter actinomycetemcomitans in diabetic and non-diabetic patients with periodontitis. Methods: 99 patients with periodontitis and healthy systemically and 105 diabetic patients with periodontitis were selected to take their Gingival Crevicular fluid samples and the PCR technique was used to detect fimA genotypes and other periodontopathogens. Data were analyzed with chi-square test and Kruskal Wallis Results: Porphyromonas gingivalis was positive in 72.7 % of non-diabetic patients and 28.6 % of diabetic patients. The most frequent genotype in both groups fimA II, being in smaller percentage in diabetic patients (P = 0.000).  There was a high correlation between positive samples for Porphyromonas gingivalis and Aggregatibacter actinomycetemcomitans and Treponema denticola and Tanerella forsythia. Conclusion: The frequency of Porphyromonas gingivalis, Treponema denticola, Tanerella forsythia and Aggregatibacter actinomycetemcomitans is lower in diabetic patients with periodontitis. The most frequent genotype in both groups was fimA II. In general, the distribution of fimA genotypes was heterogeneous, and no statistically significant differences.
