Resumen

Antecedentes: En la búsqueda de métodos confiables y de fácil acceso para predecir el remodelado óseo y la estabilidad de los implantes a través del tiempo surgen las deoxipiridinolinas (DPD). Objetivo: Comparar las concentraciones de DPD/creatinuria con la altura de la cresta ósea antes y cuatro meses después de una cirugía de implantes. Métodos: La muestra consistió en 28 pacientes sanos de 40-75 años de edad y ambos sexos, quienes iban a someterse a cirugía de implantes. La muestra se dividió en 2 grupos por sexo. Se evaluaron las DPD en orina 2 horas después de la primera micción del día, con la razón DPD/Creatinuria nM/mM (mujeres 3,0-7,4 nM/mM; hombres 2,3-5,4 nM/mM), mediante la técnica de inmunoensayo quimioluminiscente de fase sólida. La altura ósea se midió desde el ápice del implante hasta el margen de la cresta ósea, anterior del procedimiento quirúrgico y cuatro meses después con tomografía. Resultados: La edad promedio fue 53,6 años (mujeres) y 56,5 (hombres). Las medianas de DPD estuvieron en el límite superior normal (7,5 mujeres; 5,1 hombres). Se encontró correlación significativa entre los valores de DPD y sexo (p = 0,034) y entre sexo y altura ósea inicial (p = 0,010) y final (p = 0,035). Hubo correlación del 98 % significativa entre altura ósea inicial y final (p = 3,3E-12) y los promedios de altura ósea inicial y final agrupada (p = 0,005). Conclusiones: Existe alta correlación entre los niveles elevados de DPD y el incremento de la reabsorción ósea antes de la colocación de los implantes y después de ello.

Background: In the search for accessible and reliable methods to predict bone remodeling and implant stability over time deoxypyridinoline (DPD) levels emerge as an alternative. Purpose: To compare DPD/creatinuria levels with alveolar crest height before and 4 months after implant placement surgery. Methods: The sample consisted of 28 healthy 40-to-75-year-old patients, of both sexes, who would undergo implant surgery. The patients were assigned to 2 groups by sex. Urine DPD samples through solid-phase chemiluminescent immunoassay with the DPD/creatinuria ratio in nM/mM (females 3.0-7.4 nM/mM; males 2.3-5.4 nM/mM). Bone height was measured from the implant apex to the alveolar ridge before and 4 months after the surgery with tomography. Results:. Mean age was 53.6 years (women) and 56.5 (men). DPD medians were in the normal higher limit (7.5 women; 5.1 men). There was a significant correlation between DPD and sex (p = 0.034), and between sex and initial bone height (p = 0.010) and final (p = 0.035). There was a highly significant correlation (98%) between initial and final bone height (p = 3.3E-12) and the means of initial and final pooled bone height (p = 0.005) Conclusions: There is a high correlation between high levels of DPD and the increase in bone resorption before and after implant placement.
