Abstract
Objective: To analyze the uses and accents of the category hegemony in literature resulting from research on healthcare systems, published in English and Spanish between 2000 and 2022, and to identify the reference sources employed.
Methodology: Narrative literature review in PubMed and Scopus databases, as well as the Google Scholar search engine, including grey literature published between 2000 and 2022 in both English and Spanish. Studies with explicit reference to the hegemony category in healthcare system analysis were included. Definitions, cited authors, and uses of the category were systematically analyzed to produce a general understanding of its usage in academic literature. In the process of article development, the trajectory of the hegemony category, its broader conceptual framework, and its primary exponent were also reviewed.
Results: It was found that the hegemony category has different uses and accents in studies on healthcare systems, often without explicit reference to the definition or authors being cited. These uses can be classified into two broad groups: the first group makes a general reference to various coexisting and intersecting hegemonies, while the second group emphasizes the socio-political and historical processes of hegemony contention within healthcare systems.
Conclusions: The hegemony category proves to be powerful in the analysis of healthcare systems by focusing the analysis on the ethical-political level and the cultural operation of building persuasion, consensus, or domination beyond legal milestones. Studies using this category often do so superficially, reducing its analytical and explanatory power.
