Abstract
Este artículo analiza el proceso de descentralización de los servicios de salud. Basándose en
una revisión de la literatura, los autores reflexionan sobre los intentos de descentralizar el sector
salud mexicano, sus motivos, costos e impacto. El gobierno mexicano invirtió una gran cantidad
de recursos en este proceso, el cual, además, tuvo que superar muchas dificultades que no se
habían anticipado. La primera fase de la descentralización, que se inició en 1983, se interrumpió
a consecuencia del deterioro de los programas; la segunda fase también sufrió un retroceso, al
aprobarse la reforma de 2003. Después de 25 años, la descentralización no ha conseguido mejorar
la eficiencia, la transparencia en la gestión de los recursos, ni la participación comunitaria, y ha
aumentado la inequidad del sistema.
