Abstract
En 2005 el proyecto 052 se perfiló como el ganador en el proceso de la reforma del Sistema
general de seguridad social en salud (SGSSS). Durante su tránsito por el Congreso es posible la
introducción de modificaciones, pero el estudio de este proyecto permite comprender la filosofía
de la reforma, particularmente en lo que se refiere al financiamiento del SGSSS. El proyecto
continua con la compleja arquitectura del sistema, que presenta cuatro circuitos de recursos
dentro del mismo. Es un obstáculo para llevar a cabo las acciones de la salud pública colectiva
y mejorar el estado de la salud de la población. Cada uno de los cuatro circuitos presenta su
propia mezcla del mercado y de la regulación, lo que dificulta el manejo del SGSSS. El proyecto
no mejora la competencia entre los actores del sistema, e incluso introduce algunas restricciones
a la lógica del mercado, no siempre fundamentadas teórica ni empíricamente. La reforma
recompone algunas partidas entre los regímenes, lo que no pone en peligro el equilibrio del
régimen contributivo. La reforma va a aumentar los recursos del sistema, lo que debe permitir
ampliar la cobertura.
