Abstract
Los ópticas principales (libertaria e igualitarista) que subyacen en el debate ideológico sobre
sistemas de salud, son esencialmente divergentes en cuanto a la mezcla deseada de mercado y
Estado. La primera plantea que el acceso a los servicios de salud esté determinado por el libre
juego de la oferta y la demanda; la segunda propone una obligación del Estado en la distribución
de ingreso y oportunidades. El uso del análisis microeconómico no implica necesariamente
un compromiso con alguna de estas ópticas, aunque sea más útil en enfoques de mercado.
Es más bien su utilización normativa la que está determinada por las posiciones ideológicas.
Las herramientas de atención gerenciada y los desafíos a la autonomía profesional y al
principio de beneficencia señalados por los críticos de los enfoques de mercado, no son exclusivos
de éstos, sino que responden a la necesidad universal de optimizar el uso de los recursos
disponibles.
