Abstract
Este ensayo revisa las explicaciones teóricas de la integración vertical entre los eslabones de la
cadena de producción, y toma una relevancia particular para el sector salud: la inexistencia de
un eslabón de la cadena en un nuevo mercado. En la cadena vertical de los servicios de salud y
en presencia de aseguradores que enfrentan restricciones presupuestales severas, el eslabón de
coordinación es el que con mayor frecuencia está integrado, básicamente por su ausencia del
mercado. Esta hipótesis se plantea con base en la experiencia de Estados Unidos. En el caso
colombiano, podrían existir otras explicaciones de la integración vertical entre pagadores y
prestadores, pero no todas contribuirían a maximizar la función objetivo del Sistema. La integración
con el fin de extraer rentas bajando los costos a expensas de la calidad, es una posible
explicación en este sentido. Un nuevo enfoque de mercado basado en el rol de agentes informados,
sería una estrategia eficaz para separar los procesos de integración vertical que buscan
extraer rentas, de aquellos que realmente contribuyen a maximizar la función objetivo.
