Abstract
El propósito de la revisión temática fue definir unos ejes temáticos para interpretar la reforma
en salud y orientar un nuevo modelo del sistema. El método utilizado fue la revisión documental
sobre las teorías de políticas públicas, los referentes para el análisis, los cuasimercados en
salud, las redes integradas de servicios de salud (RISS) y la atención primaria en salud (APS).
Se contrastó la lógica de la salud pública (colectivo-ciudadano) y la lógica microeconómica
(individuo-consumidor). Se analizó la reforma colombiana (leyes 100 de 1993 y 1438 de 2001),
basadas en teorías del
policy choice , enfoques clásicos del ciclo de las políticas y teoría neoclásica;
la reforma estructural basada en el derecho a la salud, en la equidad y en teorías de políticas
públicas (enfoque crítico y enfoque constructivista). Se concluye la necesidad de continuar el
debate ideológico por la reforma estructural del sistema de salud; asimismo, continuar con los
desarrollos técnicos, instrumentales y de estrategias para implementar las RISS y la APS, con
el propósito de afianzar los sistemas integrados de salud.
