Abstract
Objetivo:
Realizar la evaluación económica y analizar la sostenibilidad financiera del modelo de
atención primaria en salud (APS) del Hospital de Suba II Nivel - Empresa Social del Estado (ESE).
Metodología: Se hizo un análisis de costo efectividad en el cual las unidades de efectividad fueron
años de vida ganados por reducción en mortalidad materna y mortalidad infantil por enfermedad
diarreica aguda, las unidades de costo fueron unidades monetarias, y el umbral de costo efectividad
fue el Producto Interno Bruto (PIB) per cápita. Se usaron datos epidemiológicos de Suba (2002-2003,
2005-2006), Ciudad Bolívar (2005-2006) y Bogotá (2005-2006). Se realizó también el análisis de
sostenibilidad a partir del comportamiento cualitativo esperado de ingresos y costos como resultado
de la implementación de APS y tomando en cuenta los datos contables 2004-2006. Resultados: El
costo efectividad incremental fue de $6’473.091 pesos anuales de 2006, y la implementación del
modelo en la localidad de Suba fue costo efectiva. El modelo generó 412.8 años de vida ganados
para la localidad. La implementación de APS disminuye eventualmente algunos ingresos y costos en
la misma medida. Si bien no genera pérdidas, sí disminuye la facturación. Conclusiones: La imple
mentación
de APS genera beneficios para la sociedad que, no obstante, no son susceptibles de ser
traducidos en ingresos monetarios para el hospital. Esto puede generar incentivos contradictorios en
el actual sistema, cuyos incentivos y principios de asignación se orientan hacia el tratamiento de la
enfermedad más que hacia la obtención de resultados en salud.
