Abstract
Este artículo analiza la propuesta de reforma del sistema de seguridad social en salud planteada
por el proyecto de ley 052 de 2005 en trámite en el Congreso de la República, ante algunos de
los principales problemas detectados en el funcionamiento del sistema de salud pública, rectoría
y coberturas. Los autores encuentran un contraste importante entre los logros de un sistema
que ha aumentado en forma notable el gasto público en salud y el aseguramiento y que presenta
un balance difícil en los resultados de salud pública. De la iniciativa de reforma preocupa
además que quedan sin resolver graves problemas. No se plantea un juicio sobre el aseguramiento,
pero sí sobre la forma cómo el sistema colombiano ha privilegiado objetivos económicos.
Los autores consideran que en discusiones futuras se deberán buscar salidas a temas aún
problemáticos como la cobertura, la salud pública y la capacidad de gobierno para alcanzarlas,
teniendo como norte los fines sanitarios del sistema.
