Abstract

Methotrexate (MTX) is an immunomodulatory agent used as a modifier of rheumatological diseases, it can have toxic effects mainly at the hematological, cutaneous and hypersensitivity levels that include organs such as the lung, in the form of pneumonitis. We present the clinical case of a 51-year-old female patient with Rheumatoid Arthritis (RA), who presented with Methotrexate-induced pneumonitis with subsequent improvement in her clinical status upon withdrawal of the drug and initiation of corticosteroid therapy. Different from what might be thought, methotrexate pneumonitis, being so rare, should be considered as a diagnosis of exclusion, given the similarity of the clinical picture with more common entities such as infectious and inflammatory lung diseases and autoimmune disorders.
