Abstract

Papillary carcinoma (PC) is the most common
malignancy neoplasm of the thyroid gland;
this carcinoma presents different histological variants, with other features and biological
behavior. In general, this condition has a good
prognosis, but the diffuse sclerosing variant is
less frequently and more aggressive. The incidence reported in the literature varies from 0.8
to 5.3% of all pediatric malignancies; this entity
is extremely rare in children under 5, with increasing frequency at a later age, mainly affecting
the children between 11-17 years. It is characterized by a diffuse involvement of one or both
lobes, sclerosis, abundant psammoma bodies,
prominent squamous metaplasia and extensive
lymphatic permeation. Patients with a large tumor
may become hoarse by pressure on the recurrent
laryngeal nerve, and have dysphagia, cough, or
airway obstruction. In this article, it presents a
case of a 18 years old patient who was performed
a total thyroidectomy with central emptying and
partial excision of the thymus. The pathological
study revealed that it was a diffuse sclerosing papillary carcinoma. Diagnostic methods, clinical
presentation and treatment are discussed
