Abstract

We present a case of a 66-year-old male patient with a clinical history of chronic kidney disease and obstructive uropathy. The patient required transitory hemodialysis during 5 months. The patient presented with clinical signs of a complicated upper urinary tract infection with sepsis along with KDIGO stage 3 acute kidney injury needing renal replacement therapy until recovery. Empiric antibiotic therapy was implemented without improvement and pyuria persisted. Urinary cultures were negative. The patient was taken to a left percutaneous renal nephrostomy and due to high suspicion of renal tuberculosis, a GeneExpert PCR was ordered. This case represents a rare presentation of renal infection leading to both functional and structural complications in an endemic region for TB.
